Step 1. Functional Assessment

Assistive devices are prescribed based on a functional assessment, during which the level of a person’s functional limitations is determined and an Individual Service Plan (ISP) is developed, indicating the services and assistive devices required.

 

Step 2. If you underwent a medical-social assessment before 2024 and have a valid Individual Rehabilitation Program (IRP)

 

Step 3. Apply to any territorial center of the Unified Social Service

  1. If your IRP has expired or has been lost, you need to apply to obtain a certificate confirming eligibility for assistive devices.

  2. If you belong to another eligible beneficiary category (e.g., a family vulnerability score of 28 or higher), you must provide a document issued by the relevant authority.

  3. If you have an IRP, ISP, or a certificate confirming eligibility for assistive devices, the applicant or their representative must submit an application to any territorial center of the Unified Social Service (USS) or online via the E-request system to receive a state certificate.

 

Required documents:

  • Application form

  • Identification document

  • Social Security Card or Public Services Number (PSN), or a certificate of non-receipt of PSN

  • If applying through a representative — written authorization and the representative’s passport

  • Document confirming eligibility for a state certificate (if applicable)

For hearing aids, additionally required:

  • Certificate from an ENT (otolaryngologist) confirming hearing loss and the need for a hearing aid

  • Audiometry results

For orthoses, additionally required:

  • For individuals under 18 — conclusion from a specialized medical institution

  • For individuals 18 and older — conclusion from the Unified Social Service (in case of disability)

 

Step 4. Receiving the Certificate

After reviewing the submitted documents, the Unified Social Service issues the relevant certificate(s) through the assistive devices information system and notifies the applicant in writing, also providing a list of authorized service providers.

 

Step 5. Choosing a Service Provider

The applicant independently selects a provider organization and applies there to receive the assistive device.
